To access and complete your prepopulated claim form, please check your email for two separate messages:

1. DocuSign Email

  • From: DocuSign
  • Subject: “Complete with DocuSign: Clients on Demand Claim Form”
  • This email contains a secure link to your prepopulated claim form.

2. Access Code Email

  • From: no-reply@cases-cr.stretto-services.com
  • Subject: “Notice of Clients on Demand Class Action Settlement”
  • This email contains the access code you will need to open and complete your claim form.

Important: You will need both the secure DocuSign link and your access code to access your prepopulated claim form. Please check your spam or junk folder if you do not see either email in your inbox.
